The Prognosis of Sudden Sensorineural Hearing Loss Patients Who Did Not Respond Initial Steroid Combination Therapy

Abstract
Background and Objectives : The aim of this study is to evaluate the prognosis of sudden deafness who did not respond to initial treatment including steroid, dextran, magnesium and carbogen. The author also tried to analyse the hearing recovery patterns and prognostic factors.

Materials and Methods : Clinical analysis was performed in 204 patients with sudden hearing loss who were admitted to Chonnam National University Hospital from January 2003 to June 2005.

Results: There were 114 cases (55.9%) who did not respond during initial steroid combination therapy. Among of them, there were 39 cases (34.2%) who did not recover until completion of initial treatment but recovered thereafter and the average duration from initiation of treatment to hearing recovery was about 37 days (20-83days) from initial treatment. The related prognostic factors were the time to first treatment, severity of initial hearing loss, presence of vertigo, and age.

Conclusion: After initial combination treatment of SSNHL, about 56% of patients discharged without hearing improvement. Among of them, about 34% of patients showed hearing improvement later and overall hearing recovery rate was about 63%. With the patient who did not respond to initial treatment, the duration that we could expect further hearing recovery was about 3 months after starting treatment. If necessary, we could consider other treatment such as cochlear implantation after that time.
